SUMMARY:Future Fest: Planning for Life After High School
DESCRIPTION:Join us for a transition focused gathering at the Moorhead Library. At Future Fest: Planning for Life After High School for students with disabilities\, we will have learning opportunities\, resource tables\, and more.   \nParticipants can attend breakout sessions to learn about Supported Decision Making\, tools to help plan for the future\, CCRI’s Independent by Design program\, and more. We’ll event have a creative breakout session where participants will make a collage to help them think about their future.  \nRegister in advance to attend live sessions and get a FREE meal\, or just show up for our resource fair.  \nRegister HERE \nSchedule \n\n4:30pm Session 1\n\n\n\n\nBreakout option 1: CCRI: Independent By Design \n\n\n\n\n\n\nBreakout option 2: Future Focused Collage Making \n\n\n\n\n5pm Break\n\n\n5:15pm Session 2\n\n\n\n\nBreakout option 1: Freedom Resource Center: Supported Decision Making \n\n\n\n\n\n\nBreakout option 2:Person Centered Thinking Basics \n\n\n\n\n6pm Meal and Resource Fair\n\n\n6:45pm Session 3\n\n\n\n\nBreakout option 1: Lived Experience Presentation: Successful Perspective in Supportive Decision Making with Addie and Marisa \n\n\n\n\n\n\nBreakout option 2: Putting it all together with Charting the LifeCourse \n\n\n\n\n7:15pm Resource Fair\n\n\n8:15pm End \n\nBreakout Session Descriptions \nIndependent By Design with CCRI  \nJoin CCRI to learn about their Independent by Design Program. It is an alternative to traditional community residential setting care that allows flexibility to provide services tailored to the individual. These services are designed to adapt to the increasing or decreasing needs of individuals who live in their own homes. Services available include 24-hour emergency staff response with technology and supplemental support in areas including budgeting\, menu planning\, grocery shopping\, assistance with medical appointments\, and additional areas as identified.  \n  \nPerson Centered Thinking Basics  \nLearn about the core elements of Person-Centered Thinking\, and put you and your family’s individualized support needs as the top priority as you plan for the future. We’ll discuss how to balance health and safety with the things that bring joy to your life\, and supports you might need along the way.  \n  \nSupported Decision Making  \nJoin the Freedom Resource Center to discuss Supported Decision Making\, and start thinking about tools that could help your family. Power of attorney\, health care directives\, releases of information\, joint bank accounts: The list goes on! There are many ways to help people with disabilities make decisions\, and no two support systems are the same.   \n  \nFuture Focused Collage Making  \nSometimes it can be hard to put pen to paper and plan out the future\, so we’ll be using scissors and glue sticks instead. Come get crafty with us! Participants will get their creative juices flowing while they think about the things that are important to and for them\, and their future goals.  \n  \nLived Experience Presentation: Successful Perspective in Supportive Decision Making  \nJoin Addie and Marisa as they discuss their journey in Supported Decision Making. Addie is currently in college and gets support from her mother\, Marisa in a variety of ways. Come learn what has worked for them\, and get some ideas that may help your own family.   \n  \nPutting it all together with Charting the LifeCourse  \nCharting the LifeCourse has tools to help you think about many areas of transition.
